As a Home Care Package provider, Iris Aged Care can provide permanent care at one of our two family-owned locations – Iris Manor or Hawthorn Grange. These facilities provide similar services to residential aged care homes. As they are privately operated and do not rely on Government funding, this allows us to deliver personalised and flexible care tailored to your needs in one of our locations.
Iris Manor and Hawthorn Grange offer supported accommodation for individuals who are unable to live independently and require daily support. This includes elderly residents, people with disabilities and those in need of rehabilitation following injury or illness. We are also able to offer full 24-hour support in these settings, ensuring continuous care for all residents.
Home Care Packages can be utilised to complement the services provided by these facilities, ensuring residents receive the personalised care they need. These packages also support additional services, such as specialised nursing care or one-on-one assistance, offering an extra layer of care where required.
At Iris Aged Care, we provide a comprehensive range of support services, including help with personal hygiene, meal preparation, medication management, and transport. We also recognise the importance of emotional support and social interaction, offering a variety of activities designed to promote mental well-being and keep residents connected with their community.

Frequently asked questions
A Supported Residential Service, offers accommodation and personal care for people who need help with daily activities but don't qualify or who might not want to use government-subsidised aged care. Unlike aged residential aged care homes, Supported Residential Service facilities do not receive government funding and can often provide more flexible or personalised care.
A Supported Residential Service does not require payment of a Refundable Accommodation Deposit (RAD) or Daily Accommodation Payment (DAP).
Supported Residential Services provide assistance with daily living activities, such as personal care (bathing, dressing), meal preparation, medication management and support with mobility.
Iris Manor and Hawthorn Grange provide around-the-clock care tailored to meet our resident’s unique needs.
As Supported Residential Service facilities do not receive government subsidies, the cost of accommodation and services can vary depending on the level of care required. Weekly fees cover accommodation, personal care, meals, laundry, cleaning and access to services. Additional fees for extra services are applicable where required.

Yes, even if you’re receiving 24-hour care in Iris Manor or Hawthorn Grange, your Home Care Package can be used to fund, specific services that are included in the standard Supported Residential Service care.
An example of this is that the HCP could be used to fund a portion of the personal care or nursing care provided daily. Your Home Care Package could also be used to fund allied health services, specialised nursing care, or assistance with personal tasks that require extra attention whilst you are living at one of our care facilities.
If you already have a Home Care Package, you do not need a separate assessment to use it in at Iris Manor or Hawthorn Grange. Iris Aged Care will review your needs and create a care plan that outlines the care provided by our team. If your care needs change, a reassessment by the Aged Care Assessment Team (ACAT) may be necessary.
