When you decide that you would like Iris Aged Care to provide your Home Care Package we will connect you to your personal Care Manager who will become the primary contact for your journey with us. Your Care Manager will work with you to design a service based on your needs and in consideration of your skills, capabilities to enhance your independence, health and well-being.
We strongly believe in an individual tailored approach, supporting you to increase or maintain your independence and partnering with you to ensure your needs and wishes drive the care and support planning process we provide to you.

Our service areas
Iris Aged Care is located in Ashburton, Victoria. We manage Home Care Package delivery and services for clients primarily in the South Eastern suburbs of Melbourne including Camberwell, Kew, Hawthorn, Toorak, Malvern, Chadstone, Box Hill and surrounding areas.
Iris Aged Care owns and manages two Supported Residential Service facilities. Iris Manor located on High Street in Ashburton and Hawthorn Grange which is located on Hunter Street in Hawthorn. As these are Supported Residential Service, private care facilities, they are considered your private home and as such your Home Care Package can be delivered at either location.
